Our Privacy Policy was developed to further our commitment to combine quality products and services with integrity in dealing with our Users. Our Policy is designed to assist you in understanding how we collect, use and safeguard the personal information you provide to us and to assist you in making informed decisions when using our site and accessing our products and services.

What Information Do We Collect?

When you visit our site you may provide us with two types of information: personal information you actively choose to disclose ("Active Information") and information collected by us in a way not visible to you, on an aggregate anonymous basis as you and others browse our site ("Passive Information").

Active Information You Choose to Provide

In order to gain use of the site (become a "User"), we require you to provide us with the following information: Full legal name, address, phone number(s), age, blood type, and at least one (1) emergency contact person, including his/her full legal name, address, and phone number(s).

We use secure socket layer (SSL) encryption to protect the transmission of the information you submit to us when you use our secure online forms. The information you provide to us is stored securely in our database.

If you agree to use our service and post your Emergency Health Profile on our site or maintain email, postal mail or telephone contact with us, we may retain such information for as long as we deem appropriate unless you provide us with a written notice of Revocation by mail or through our web site. Upon receipt of a written notice of revocation we will delete your Emergency Health Profile from our database; however, we may retain your name, address, email address and phone number unless you specifically ask us in writing to delete that information as well. WARNING: TRANSMISSION OF YOUR MEDICAL INFORMATION TO US BY E MAIL WITHOUT THE USE OF OUR SECURE ONLINE FORMS MAY NOT BE FULLY SECURE.

What is a Cookie?

Cookies are a feature of web browser (Internet Explorer, Netscape Navigator, etc.) software that allows web servers to recognize the computer used to access a site. They are small pieces of data stored by a recipient's web browser to simplify subsequent interactions with the same user or to streamline the user's transaction on related web pages. Our site utilizes cookies to collect Passive Information about how our site is used. Broadly speaking, persons whom we employ or contract with use Active Information for purposes of performing our service to you, providing customer services such as technical support, and making available other products or services we think may be of interest to our Users. We may use the Active Information or Passive Information to contact you about changes to our site, new services, features or products we offer, or other information we think you shall find valuable. We use Passive Information to gather information about our Users and to enhance and design our site to make it easier, faster and friendlier to use. Additionally, cookies help us know information about how many people visit our site, when they visit and how they use our service. Should we enter into a transaction with another company, that other company may need to review our company's assets in a confidential manner, including your Active and Passive Information, to make business decisions as to whether they wish to enter into a transaction with us.

Sharing Information with the Government or As Otherwise Required by Law

We may be required by subpoena, law or government agency to disclose both Active and Passive Information you have provided to us, and we will follow all legal requirements to disclose information we have obtained from you. We also may have to disclose such information to governmental investigators or police officers and you hereby authorize all such disclosures.

How Do We Secure Active Information and Passive Information?

We secure your Emergency Health Profile submitted by you by using the method described in this User Agreement to prevent unauthorized access or disclosure, or accidental loss or destruction of Active Information and Passive Information. Your postings on this site and other communications you may have with us via email or postal mail may not be secure unless we advise you that security measures are in place prior to your sending information. Therefore, if you choose to communicate with us through email or postal mail, you are assuming the risk of doing so, and we request that you do not send sensitive information through these unsecured means.

The information that you send to us in a secure fashion, as described in this User Agreement, will be protected by the encryption process described above. Understanding your health Information

Each time you visit Medical Envelope you are able to create, update, or delete your Emergency Health Profile. You may send information about your symptoms, examinations, test results, medications you take, your allergies, and your plan for care. The creation of your Emergency Health Profile, using the information you decide to disclose to us, is an essential part of the service we provide for you.

Uses and Disclosures of Health Information

Medical Envelope is in the business of providing your Emergency Health Profile to "First Responders", such as police or fire departments, ambulance companies, physicians, nurses, emergency departments, EMS, schools, disaster control authorities, sports organizations, etc.

We will use your Emergency Health Profile for our regular business operations. For example, Managers and staff of Medical Envelope may review your medical information from time to time to complete a quality review of our services to determine how our service can be improved.

Other Disclosures

Communication with others
In an emergency, we may notify a person you have identified as an emergency contact to advise them of the emergency provided that we are able to contact the person.

Refund Policy

It is Medical Envelope's intent to provide a service of value and 100% satisfaction. If you are not 100% satisfied with your subscription to our service you may request a full refund within 30 days of your purchase.

Renewal Policy

All subscriptions to Medical Envelope's service will automatically be renewed at the end of the payment year. However, you may send a written request to Medical Envelope within 30 days of the renewal of the subscription for cancellation and a full refund.
